PHIAC 3 and PHIAC 4 forms now available on PHIAC’s site
 
Circular 09/14 advised that data for the Act would be collected in the PHIAC 1, PHIAC 3 and PHIAC 4 forms from the December quarter 2009. The PHIAC 1 form, revised to include the ACT, was included with circular 09/14. This Circular is to advise that revised PHIAC 3 and PHIAC 4 forms, on which ACT is to be reported separately, are available from PHIAC’s web site on the Forms page. NSW statistics will no longer include ACT.
 
 
Clarification of reporting of membership changes during the quarter on the PHIAC 1 form for the December 2009 quarter
 
Insurers should note the requirements for start of quarter policies and persons in the section “Changes During the Quarter” on page 2 of the PHIAC 1 form for the December 2009 quarter.
 
Start of quarter policies and persons for NSW on the December 2009 PHIAC 1 form should include ACT policies and persons as reported at the end of the September 2009 quarter.
 
Start of quarter policies and persons for ACT on the December 2009 PHIAC 1 form should be zero.
 
The change in reporting from NSW to ACT, for the NSW data, should be reported as “Transferring to another state”.
 
The change in reporting from NSW to ACT, for the ACT data, should be reported as “Transferring from another state”.
